Medicare usually covers 80% of pocketed bras and forms with supplemental insurance picking up the remainder. Contact your insurance provider to learn the details of your plan.
Forms/prosthetics can be replaced every 1-2 years [depending on insurance plan]
Bras may be replaced
as often as every 3 months.

The Women's Health and Cancer Rights Act of 1998 guarantees that insurance companies not only cover cost of mastectomy but also reconstruction, if chosen:
Reconstruction of the breast that was removed by mastectomy
Surgery and reconstruction of the other breast to make the breasts look symmetrical or balanced after mastectomy
Any external breast prostheses (breast forms that fit into your bra) that are needed before or during the reconstruction
Any physical complications at all stages of mastectomy, including lymphedema [fluid build-up in the arm & chest on the side of surgery]

Keep itemized receipt from your purchase to send to insurance company.
Call your insurance company for more information on their filing procedures
